Follow up with the Dr's office was a struggle because they had not heard anyone ask for receipt with details of insurance company codes.
Insurance companies give a code to each service. So immunization for something would be IMZ1234. The Dr had to provide how many injections with code, x rays with code etc. You then send it to the insurance company and get the refund.
Another way could be you can get the information before hand from the insurance company and then go for medical.

But remember if this advice helps and you get that refund donate it to IV.

ITIN numbers are for tax filing purposes only. That's what it says when you get your ITIN card. I dont know if you can use this for any other purposes.
You can use ITIN no. as a substitute for SSN for most of the places except employment. Its true for schooling too.

Dear IV members, I would like to share my GC story with u. I have been in the US since 2001 mostly on and off. I wasnt sure whether I want to permanently live in America and thats why I havent even thought about getting a green card before. Last year I made up my mind and applied for an employment based GC. I started my labor on January 1, 2008 (first ad in the local newspaper) and labor was applied in Fabruary. BTW I am EB2 (rest of the world). My Labor was approved in the end of April. Conccurently filed I-140 and I-485 along with AP and EAD in mid May at TSC (RD May 14 and May 20 respectively, FP done June 28 and no LUDs whatsoever after that, was a little upset over that). My birthday was on Aug 19 and what a huge present I had. I still cannot believe my luck. First on Aug 18 EAD approval followed by my I 140 approval on Aug 22 and 2 hours later I received the magic email: I-485 current status: Card production ordered!!!!!!! Unbelievable! I couldnt believe my eyes. I logged in a logged out of the USCIS website 3 times to make sure that it is not a mistake. 3 days before the approval notices I was talking to my parents telling them that I will probably wait for another 1-2 years and then I got it! No RFE no questions whatsoever. After I saw the email I felt such a relief and sense of freedom. Later in the day I spoke with my lawyer and he couldnt believe that the 485 was approved. To be honest with u I didnt even know the whole process of getting a green card (lawyer was taking care of it) before I stumbled on the Immigration Voice website after a google search. I have followed all of the threads for the last 7-8 months here and I find them extremely helpful and a great source of information. Sorry for the long post and I wish you all to get your green cards from the bottom of my heart. My 8 months are nothing compared to some of you who have been waiting for years so my advice is to spend a lot of money on a good lawyer and hope for the best, the rest of it is in God's hands. With the current wait time, it is almost impossible to survive on the job till we get the green card. If one loses job before I-485 application, all green card efforts are down the drain, and he/she becomes instantly out of status without work on H1B.
However, if it is allowed to file I-485 based on some qualifications (i.e. I-140 approved for more than a year, etc.), we can stay legally with AOS without having to worry about losing jobs.
We had a similar discussion in March which ended up with no action items. This time I want to enroll as many people as possible and we will co-sign a letter that will be sent to USCIS, DOS, and Congress members. Please reply this thread if you'd like to have your names included with signature for this petition. If you have other ideas that will help us, please feel free to use this thread to contribute.
Few members started initiative for same. But I think Its not possible. Pray to GOD that we may get chance. What else can be do ? We are minority with in minority. Also we cannot qualify for human right !!!!

I can't tell from your question whether your current employer filed for H-1B status for you. If you are not in any valid nonimmigrant status now, you need to file for permanent residence (PR) via marriage ASAP. Even if you are out of status, you entered the US legally so you should be able to file for PR via marriage (see blog link (http://martinvisalaw.blogspot.com/2009/03/my-spouse-is-illegal-what-can-we-do.html) for details).
If you have valid H-1B status for your current employer, you can also file for PR even if the process has already started via employment. This is common, especially since it is usually a lot faster via marriage.
You may have been out of status for the 4 months that you were not working. It's hard to say exactly, without know what your current status is and whether there was any gap.

My I485 is filed at NSC and my both I-140's(eb3 and eb2) are at TSC. Will it be a problem?
How would NSC know my I-140(Eb2) is recently approved from TSC?
Did anybody experience this?

This will delay the I-485 approval. You/your attorney can send a letter requesting USCIS to adjudicate your pending EB-3 I-485 based on EB-2 I-140 approval. NSC will request the transfer of your EB-2 I-140 records from TSC to NSC and an IO will then have to go through your case and approve it. This process took approximately 45 days for my colleagues whose EB-2 I-140 was approved at TSC end of July 2010 and his I-485 which ws pending at NSC was approved second week of Sep 2010.

This is all due to their better understanding of what is a Receive Date. So far, they have been treating date when they physically enter data in the system (date which you see online as "we received your case on...") as the Receive Date, and making all predictions, postings and claims based on that. Now they know that it is what you see on your receipt as the Receive Date, and hence the back step in the dates. TSC is at June 18, NSC is at July 2.
They should better post where they are based on PDs, and work based on that too.
Now that the dates have moved back to Jul 02 for NSC, any guess as to when they would pick Oct ND cases?.

After waiting for 95 days, we got out approved AP's today in the mail. The online status still says received and pending. Me and my spouse only had one LUD and that was on the 4th of September.

It is good idea to invest 10% in gold. I would recommend to buy physical gold. Paper OR Virtual gold is not useful. Because if you buy Virtual gold and later on if you want to get physical gold (Some companies give an option to convert to physical gold with some % charges), company may charge too much money (especially when gold prices are high OR gold scarcity). So it is better to buy physical gold unless you want to buy gold for short investment (Speculation business). BUT NEVER MAKE JEWELLERY OUT OF GOLD IF YOU BUY FOR INVESTMENT. Once you make jewellery, it is no longer an investment (IMHO).
Gold has hit record highs and is predicted to go even higher(weather the economy goes south or north). Experts recommend that gold should be as much as 10% of your portfolio.
Does any one have any good experience buying gold bullion? Did you buy physical gold or did you buy the virtual gold (like the one Canadian mint allows).
I am looking for any tips and advise from the experts.

If i travel back to US on AP, i know that my I94 will be Stamped Parolee.
My question How long this I94 will be valid. My I94 expires in Sep 09. So will it be given only till Sep09.

I had similar experiences in other states.. I got new license in California 2 months back and it was super smooth... They didn't ask for anything.. May be I was lucky or may be because I had CA license 6 years back... if they had asked for H1/H4... I would've got it for a year max.. Now I had it for 5 years... Not sure how they'll treat renewal...
In prev states, I was living on annual license for last 2 years...
